Physical Therapist (Morgan City/Franklin)

Details: The Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well being, and evaluates the patient's progress.   Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice. Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals. Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes to the agency at least twice a week. Completes all patient evaluations and develops the PT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to supervising nurse within 24 hours of the evaluation. Participates in staff conferences and committees as necessary. Provides supervision of the PTA as per LHC policy. Prepares a written discharge summary and/or a written summary report on patient's condition at least every 60 days. Provides in-service education as requested by DON or Branch Manager.

Occupational Therapist - Home Health(Morgan City/Franklin)

Details: The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. He/she interacts and practices in collaboration with a variety of professionals, including physicians, nurses, educators, social workers, occupational therapists, and speech-language pathologists. The therapist will also provide appropriate education to patients and or family.1.Manages patient care by first completing an evaluation, establishing a treatment plan with objective and measurable goals (outcomes), and provides direct intervention or supervision thereof .2.Performs therapeutic procedures ( exercise, neuromuscular reeducation, gait, massage, manual therapy, orthotic fitting and training, prosthetic training, dynamic activities, wound care, compression garments).3.Utilizes exercise equipment ( treadmill, ergometers, leg-press, isokinetic, inertial system, fitter, ankle platform system, pulley system).4.Performs modalities ( moist heat, ice, mechanical traction, electrical stimulation, vaso-pneumatic devices, paraffin, hydrotherapy, fluidotherapy, ultraviolet, iontophoresis, ultrasound).5.Delegates tasks and duties as indicated.6.Participates in continuing education, departmental meetings, and in-service programs.7.Adjusts patient care schedule and prioritizes tasks as indicated.8.Performs documentation per policy and procedures ( evaluation, progress notes, discharge summary, and multi-disciplinary education and treatment plan forms).9.Performs patient and family education regarding interventions, disease/injury, safety, and wellness.10.Participates in quality improvement initiatives.11.Participates in discharge planning with the multi-disciplinary team.12.Maintains a clean and orderly work area.13.Completes required documents for supply and treatment charges.14.Participates in clinical supervision/instruction of students.15.Complies with all departmental and facility policy, procedures, and directives (i.e. role delineation and scope of care).16.Performs other duties as assigned.
