Details: Celebrate Caring at Elms Haven Care and Rehabilitation Center, a SunBridge Healthcare affiliate. We're seeking a full time Social Worker at our 242-bed skilled nursing center located in Thornton, CO. Our center offers Rehab Recovery Suites and Solana Unit. As our Social Worker, Primary responsibility is to and/or assist with and participate in the planning, organizing, developing, of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations; Company established policies and procedures. The focus is to assist each resident and family in adjusting to placement, illness, and plan of care to attain the highest practicable level of functioning.
